Description

DSS/HRA goals and objectives for this RFx are to provide emergency and on-call case management services to victims affected by a large-scale disaster or other emergencies. Case managers would facilitate the delivery of appropriate resources and services. The selected proposers would work with disaster survivors to develop and carry out a client-specific recovery plan with the goal of stabilization and a return to normalcy. This partnership would facilitate client access to a range of resources. The services to be provided may include an assessment of the client verified disaster-caused unmet needs; development of a goal-oriented plan; organization and coordination of information on available resources; monitoring progress toward reaching the recovery plan goals; and, when necessary, client advocacy. Clients may access services by visiting DASCs, Mobile Outreach Units, or other facilities established by the City. Depending on the size and scope of the disaster, Mobile Outreach Units may be required to canvass the city for those who are unable to reach Restoration Centers.
