Description

This is a notice that New York City Health Department is seeking comments from the public about the proposed 2 (two) contracts below.

 

Contract Type: Contract

Scope of Services: The contractor will provide and expand access to a Nurse Care Manager Model of Health Care by reducing the morbidity and mortality related to alcohol and substance use among New Yorkers. Contractors will develop and maintain their capacity to comprehensively identify and address the holistic health needs of people who use drugs (PWUD) and reduce substance use-related harms. This will be accomplished through the utilization of a nurse care manager-led collaborative care approach with the integration of low-barrier, medication-first, patient-centered, and harm reduction principles and approaches to engagement in care.

 

Term: July 01, 2026 through June 30, 2035

Renewal Clauses: No Options to renew.
