Description

The DOC seeks qualified vendors to implement a jail-based program model utilizing the Demonstration Project Challenge-Based Procurement Method that will: • Provide comprehensive, individualized transition planning services, including assistance with housing, employment, obtaining identification, accessing public benefits, and connecting to community-based resources. • Deliver reentry-focused services on topics such as job readiness, interviewing skills, financial literacy, resume preparation, as well as attitudes and beliefs. • Offer targeted support for special populations, including emerging adults, women, veterans, individuals with mental health needs, and LGBTQ+ individuals. • Ensure a seamless transition from jail to the community by affording transportation to community-based organizations and transitional housing for sentenced individuals and detained individuals, whenever possible. • Develop a strategy to provide effective discharge planning to detainees, whose release dates are usually unknown, including a connection on the day of release. • Support individuals sentenced to state prison through their transition process. • Provide enhanced wraparound services to individuals housed in specialized reentry-focused housing areas. • Provide tailored transition-planning services to LGBTQ+ individuals, preferably in partnership with an organization that specializes in serving this population. • Collaborate with external reentry partners to create a continuum of care that extends beyond incarceration.
