Description

FY25 RQM THE MENTAL HEALTH ASSOCIATION OF NEW YORK CITY INC  Geriatric ACT team will provide support to individuals with Severe Mental Illness (SMI) by providing intensive outpatient treatment in vivo' in their community of choice. The program facilitates recovery and increased stability including symptom reduction and their impact on daily functioning and quality of life. The Service Dollars will address the immediate needs of a recipient.
