The Contractor shall provide Survey Monkey licenses, it is an online application for the agency staff to create and conduct survey, polls, quizzes and questionnaires. Conducting surveys is an unbiased approach to decision-making.  This vendor will provide the following:  DOHMH purchased 150 enterprise licenses but needs to cover and covert existing paid accounts and DOHMH user's personal account using health department email to agency enterprise accounts, either paid or free. Over 6 months analysis, we found we can convert 150 enterprise licenses into 365 licenses to cover all existing and outstanding accounts for DOHMH (65 enterprise/Power User licenses and 300 standard user licenses).  The overall cost of this contract and the yearly cost will not change.
