In NYC Health + Hospitals’ patient population of over one million, food insecurity is one of the most frequently cited needs. However, accessing relevant and meaningful food resources in New York City can be challenging, particularly for patients with technology, language, and literacy barrier. In 2021, NYC Health + Hospitals launched the Food Navigator program with the aim of providing an innovative one-stop shop that easily and reliably connects patients experiencing food insecurity to relevant and appropriate resources. The program is the largest in scale of its kind and offers connection to a broad range of resources, such as SNAP assistance, emergency food, home-delivered meals, and tailored programs to manage chronic disease. NYC Health + Hospitals seeks a vendor to continue to operate a hybrid on-site/virtual Food Navigation program with capacity to serve a minimum of 8,000-8,500 patients annually for three years. Food Navigators will assist patients referred to them by clinical providers and health system staff through screening, assessment, and connection to food resources in the patient’s community. The Social Determinants of Health team at NYC Health + Hospitals will serve as the central point of contact for the Food Navigator program for system-wide operations, performance management and reporting, capacity management, and overall strategy. At the same time, Food Navigators will collaborate with care teams and facility-based staff at various referral points.
