IN THE MATTER of a proposed contract between the Department of Veterans Services and Unite USA, Inc., 217 Broadway, Suite 800, New York, NY 10007, for the provision of the Unite Us Technology Platform consisting of service providers through a referral system for care coordination center services. The proposed contract is in the amount of $525,000.00. The contract term shall be from October 1, 2020 to September 30, 2023 with three one-year renewal options from October 1, 2023 to September 30, 2024, October 1, 2024 to September 30, 2025 and October 1, 2025 to September 30, 2026.
