NOTICE IS HEREBY GIVEN that a Contract Public Hearing will be held on Thursday, May 30, 2019, at 1 Center Street, Mezzanine, Borough of Manhattan, commencing at 10:00 AM on the following:

 

IN THE MATTER of a proposed contract between the City of New York Department for the Aging (DFTA) and Services and Advocacy for Gay, Lesbian, Bisexual and Transgender Elders, Inc., located at 305 7th Avenue, 15th Floor, New York, NY 10001, for operating DFTA’s Caregiver Services program, Citywide.  The contract term shall be from July 1, 2019 to June 30, 2020 with one two-year renewal option from July 1, 2020 to June 30, 2022 and one three-year renewal option from July 1, 2022 to June 30, 2025.  The contract amount will be $500,000.00.  E-PIN #: 12519I0001005, PIN #: 12520CARE6K3.

 

The proposed contractor has been selected through HHS ACCELERATOR, pursuant to Section 3-16 of the Procurement Policy Board Rules.

IN THE MATTER of a proposed contract between the City of New York Department for the Aging (DFTA) and Hartley House, Inc., located at 413 West 46th Street, New York, NY 10036, to support the Home Outreach Program for Elders (HOPE) which serves the elderly population with critical services to allow seniors to “age-in-place” and remain connected to the community. The program will be serving Community District 7 in the Borough of Manhattan.  The contract term shall be from July 1, 2018 to June 30, 2019. The contract amount is $197,000.00.  E-PIN #: 12519L0208001, PIN #: 12519DISC313.

 

The proposed contract is being funded through City Council Discretionary Funds/Line Item Appropriation, pursuant to Section 1-02 (e) of the Procurement Policy Board Rules.
