Extension procurement method pursuant to Section 3-04 (b)(2)(iii) of the Procurement
Policy Board (PPB) Rules.

Human Resources Administration of the City of New York, 150 Greenwich Street,
37th Floor, New York, N.Y. 10038 on business days, from June 10, 2015 through
June 23, 2015, between the hours of 10:00 a.m. and 5:00 p.m., excluding, Saturdays,
