NOTICE IS HEREBY GIVEN that a Contract Public Hearing will be held on Thursday, November 14, 2013, at 125 Worth Street, 2nd Floor Auditorium, Borough of Manhattan, commencing at 10:00 AM on the following:

 

IN THE MATTER of a proposed contract between the Department of Health and Mental Hygiene and the Contractor listed below, for NY/NY III Congregate Supportive Housing Program – Population I – Chronically homeless single adults who suffer from a serious and persistent mental illness (SPMI) or who are diagnosed as mentally ill and chemically addicted (MICA).  Population III – Chronically homeless single adults who have a substance abuse disorder that is a primary barrier to independent living and who also have a disabling clinical condition (i.e. a medical or mental health (non-SPMI) condition that further impairs their ability to live independently)  Population V – Chronically homeless families, or families at serious risk of becoming chronically homeless, in which the head of the household suffers from a substance abuse disorder, a disabling medical condition, or HIV/AIDS. The term of this contract shall be from January 1, 2014 to June 30, 2016 and contains two three-year options to renew from July 1, 2016 to June 30, 2019 and from July 1, 2019 to June 30, 2022.

The proposed contractor was selected by means of the Competitive Sealed Proposal Method, pursuant to Section 3-03 of the Procurement Policy Board Rules. 

 

A draft copy of the proposed contract is available for public inspection at the New York City Department of Health and Mental Hygiene, Office of the Agency Chief Contracting Officer, 42-09 28th Street – 17th Floor, Long Island City, NY 11101, from November 12, 2013 to Novemberr 14, 2013, excluding Saturdays, Sundays and Holidays, between the hours of 10:00 AM and 4:00 PM.
